For new tablet owners, the attached form must be signed and submitted to your branch manager for your personnel file. Please download, print and sign this form.GentivaLink Device Acknowledgement Form
Transferring a GentivaLink Tablet to Another Clinical AssociateSubmit an ESS ticket for Gentivalink Tablet and select Device Transfer. Our IT team will transfer and wipe the device back to original settings. Once we complete this process, you can follow the steps to re-activate the device Go to Employee Self Service

My Tablet was lost or stolen. What do I do?
To report an tablet lost or stolen, Call the Helpdesk at 1-800-944-HELP(4357)Our IT team will wipe the device of all Gentiva data and send Risk Management and Compliance a notification. Please make sure to notify your branch manager so accommodations can be made to replace your device.
